  2. Make sure you sign the application. Have your parent or guardian sign if necessary.
  3. If required, enclose a check with your application for the correct amount, payable to the college.
  4. If the college does not provide one, you may wish to enclose a self-addressed postcard in the application which will be mailed back to you indicating that the application has been received by the college admissions office. It is also advisable to keep a copy of your completed application for your personal records.
  5. Enclose neatly typed or written pages if needed for essay, or if necessary to answer questions. Place your name and Social Security number on each extra sheet of paper included in your application.
  6. Follow your application instructions carefully. If appropriate, mail your portion of the application directly to the Office of Admissions of the college, and submit the "Secondary School Report" portion to your counselor. If a transcript is required, complete the REQUEST FOR TRANSCRIPT form. Your counselor's recommendation is usually included with the "Secondary School Report", so it is important to follow procedures outlined for requesting a recommendation.

ADDITIONAL INFORMATION AND DEFINITIONS:

THE APPLICATION is a form provided by the college which you must complete. Some colleges provide a specific section called the Secondary School Report (this is to be completed by your counselor). The application may also include personal information, etc. It is each student's responsibility to complete the application form neatly and correctly. Sometimes the college instructs you to send the application directly to them, other times you are asked to submit it to your counselor. Please read the instructions on each application carefully.

THE TRANSCRIPT is a copy of your high school record. It is the high school's responsibility to compile this information and send it to the college. YOU MUST COMPLETE THE FORM WHICH IS AVAILABLE AT THE FRONT COUNTER IN THE COUNSELING CENTER. NO TRANSCRIPT WILL BE SENT WITHOUT YOUR PERMISSION IN WRITING. Please note that the colleges will be notified of any changes in your Grade 12 courses.

SPECIAL NOTE: The earlier you submit your application and/or recommendation to the Counseling Center with complete information, the earlier it will be processed and mailed to the college. With the number of applications, transcripts and/or recommendations which need to be prepared, it may take three weeks or longer after you submit all materials, before the total information is completed and mailed.
