Make checks payable to PVPUSD or fill out the credit card information on the registration form.

• Return your completed registration form with the attached payment to your school office, the Enrichment office at 38 Crest Road West, Rolling Hills, or FAX it to (310) 265-5967.

• A $25 processing fee will be charged for the returned checks.

PROGRAM POLICIES
• The enrollment form must be received in the Enrichment office at least 24 hours prior to the first class meeting.
• Once enrolled in the current session, you can add a class by calling the Enrichment office. We will add the class to your existing enrollment form.
• Children may not attend class until we receive a completed registration form and all fees are paid. This includes any outstanding Kids’ Corner or district related fees.
• There are no make-up classes and no refunds for absences.
• There are no student substitutions, or transfers to other classes or school sites.
• No confirmation will be sent.
• There will be NO REFUNDS after enrolling.
• Classes will be canceled if they do not meet minimum enrollment. If a class is canceled students will receive a full refund.
