Benefit Information

 

The PVPUSD offers a comprehensive benefit package to full time employees. Medical, dental, and vision plans are offered. The District also offers life insurance, IRC Section 125 plans, TSA 403(b) and 457 participation.

Dental & Vision
 

The “Open Enrollment” period for Dental and/or Vision Plan is September 1st through September 30th, 2011, with changes effective October 1, 2011.  You may add coverage or dependents during this period.  Dependents may be deleted anytime.The “Open Enrollment” forms are available below and also in Human Resources, Health Benefits.  Return form to Trish Rangel, Employee Insurance Benefit Technician, at District Office by September 30th for dental and vision changes.

   Medical

The “Open Enrollment” period for Medical Plans through CalPERS is October 10th through November 4th, 2011, with changes effective January 1, 2012. Please complete Open Enrollment form for medical from the PVPUSD website and include the same documentation as above for dependents.  Medical forms must also be returned by Friday, November 4, 2011 to the Benefits office.

If you are selecting a Blue Shield HMO/Blue Shield Net Value it is advisable to designate your physician. CalPERS has created a website; “The Chooser” which helps you to select the health plan that’s right for you.  Visit www.calpers.ca.gov to use the Chooser.
